By signing above, you  are in agreement with Perennial Kennel, and its assigns,  in overall care of your pet, during the length of stay, stated above.  
In the event of an emergency, you agree to hold harmless Perennial Kennel, Marta & Harry Alfred, and every  effort will be made to contact you, for your further instructions,  or enable them to act in your behalf, in regards to the treatment of your pet.  
The attending veterinarian will expect to charge your credit card, for no more than the initial quote, and extends to Perennial Kennel,  the ability to make a decision, in regards to the care of your pet,  should you not be available for consult.  
All efforts will be made to care for your pet, to the best of the ability, of those involved.
Veterinary care is solely the responsibility of the owner listed above.

Dogs will be considered abandoned if left 24 hours beyond pick up date, unless Perennial Kennel  is previously notified of change.  
In the event of the above,  the stated owner  therefore relinquishes all rights to  the dog, and a new home will be found, either through local rescue, or the Humane Society.
